NHS Blunder Leads to Tragic Death of 27-Year-Old Man After Receiving Wrong Covid Vaccine, Reports Say

Jack Last, a 27-year-old engineer from Stowmarket, Suffolk, received the Oxford AstraZeneca vaccine due to errors in his medical records, a newly released report has revealed. Last was mistakenly identified as living with his “at-risk” parents, which led to him receiving the vaccine earlier than he should have.

Jack received the jab on March 30, 2021, and began experiencing severe headaches soon after. Tragically, he died three weeks later, on April 20. Just a week after Jack’s vaccination, government health advisors recommended that people under 30 receive alternative vaccines, such as Pfizer or Moderna, due to a higher risk of fatal brain blood clots linked to the AstraZeneca vaccine.

A report from the Suffolk and North East Essex Integrated Care Board found that Jack’s invitation for the vaccine was due to outdated information. His medical record still listed his parents’ landline number, which led authorities to incorrectly believe he lived with them and qualified for the vaccine based on their at-risk status. In reality, Jack had moved out in 2018 and updated his contact details, but the old landline remained on file.

The Daily Mail report further indicated that Jack’s vaccination was also triggered by a mention of COPD in one of his parents’ records, even though the condition was no longer active. Days after receiving the vaccine, Jack began to feel unwell, eventually contacting 111 for medical advice.

Despite seeking help, Jack’s CT scan, outsourced to a separate company, failed to detect any brain abnormalities. A CT venogram the following day confirmed the presence of a blood clot, but the delay in treatment by 15 hours was a missed opportunity, although the report concluded it likely would not have altered the outcome.

Jack’s family described the findings as “heartbreaking,” learning that a series of errors had led to his early vaccination. The family’s statement also reflected their pain over the unnecessary invitation for Jack to receive the jab.

Dr. Andrew Kelso, Medical Director of the Suffolk and North East Essex ICB, offered condolences, saying, “We are truly sorry for what has happened and for the loss, heartbreak, and distress they must be experiencing.” An inquest in 2022 ruled that Jack died as a “direct result” of the vaccination.
